Nursing SOAPIE Note Template

SOAPIE extends the familiar SOAP structure with the two steps nursing actually turns on: Intervention, what you did, and Evaluation, whether it worked. Across two pages this template carries subjective report, a vitals block, assessment findings and labs, the nursing assessment, the plan, timed interventions, and an evaluation section that asks for the patient's response and the next step.

It also includes an escalation block for provider notification when the response was inadequate, and an attestation line for signature, credentials, and co-signature. The legal guidance is printed on the page: date, time, and patient identifier on every entry, late entries labelled and signed, and corrections made with a single line-through that leaves the original readable.
