The sick visit is the other half of a pediatric day, and it carries two things an adult SOAP note does not. First, almost every prescription is weight-based, so the header asks for the dosing weight in kilograms and the plan asks for the mg/kg/dose alongside the calculated dose. Second, hydration and intake are often the assessment, so the subjective section prompts for oral intake, wet diapers or voids in 24 hours, and vomiting or diarrhea, and the exam prompts for general appearance and hydration status before anything else.
